2. To get started enter your SSN, then enter your first and last name and your date of birth.
3. Complete your benefit election preferences and submit the required information through the secure online portal.

Paper Forms

1. Please call toll-free (844) 446-9472 to request a copy of your forms by mail or email. To verify your identity, you will be asked to provide your first and last name, the last 4 digits of your SSN and your date of birth.
2. Once you receive your forms by mail or email; enter your first and last name, full SSN, and date of birth; complete your benefit election preferences; and hand-sign the form (electronic signatures are not accepted).

Completed forms may be submitted via secure/encrypted email to myira@penchecks.com, or by fax or mail.
