Did you receive a benefit election notice or welcome letter from PenChecks?
There are two ways you can request a distribution of your funds:
Online
- For fastest service, go to the following secure site: mybenefitelection.com
- To get started enter your SSN, then enter your first and last name and your date of birth.
- Complete your benefit election preferences and submit the required information through the secure online portal.
Paper Forms
(Note: Paper forms require longer time to process)
- Please call toll-free (844) 446-9472 to request a copy of your forms by mail or email. To verify your identity, you will be asked to provide your first and last name, the last 4 digits of your SSN and your date of birth.
- Once you receive your forms by mail or email; enter your first and last name, full SSN, and date of birth; complete your benefit election preferences; and hand-sign the form (electronic signatures are not accepted).
Completed forms may be submitted via secure/encrypted email to myira@penchecks.com, or by fax or mail.
