Did you receive a benefit election notice or welcome letter from PenChecks?
There are two ways you can request a distribution of your funds:
Online
- For fastest service, go to the following secure site: mybenefitelection.com
- To get started enter your SSN, then enter your first and last name and your date of birth.
- Complete your benefit election preferences and submit the required information through the secure online portal.
Paper Forms
(Note: Paper forms require longer time to process)
- Please request paper forms by submitting a request here.
- Once you receive your forms, enter your first and last name, full SSN, and date of birth; complete your benefit election preferences, and hand-sign the form (electronic signatures are not accepted).
- To securely upload your completed benefit elections, please use our Secure Portal.
